Please read and follow these steps carefully to be fully registered.

All Athletes must have the following forms on file with the Akron-Westfield Athletic Office before they will be allowed to participate.

Step 1:  Click the Athletic Team Registration icon above. If you do not currently have an account, you will have an oppurtunity to set one up after clicking the icon above.

Note Step 2 Activity: Please select the correct sport and correct grade level. If you accidentally make a mistake, you will have to rereigister that student again for the correct sport. You will not have to reenter all the information again, just check the correct sport and grade level. Also please delete the mistaken sport and grade level.

Note: Athletes must have a current student Physical Examination form and Parent Permission/Concussion documentation on file before the student is authorized to participate in practice/tryouts. To ensure clearance for participation, we recommend all necessary paperwork be submitted at least 3 days prior to the first day of practice. 

Physicals are required to be updated yearly.  The Parent Permission/Concussion documentation must be updated at the start of each new school year, which is after July 1st. Parents/Guardians and students will need to review documentation throughout the registration process and will need to provide electronic signatures. We recommend this be completed with each student present.

Step 2: Please fill in the necessary information online.  In addition, you will need to upload the completed Physical Form when you register for a sport and/or when the athlete's current physical is expiring (Step 4 - Physical Forms).  Once this is uploaded, school staff will review the form to verify completion and dates.  You will NOT need to submit a hard copy of the physical form to the school in addition to the upload.

Once you have your physical completed by a doctor, please make sure the physical form is completed entirely and that both parent and physician have signed before uploading.

Note: If you miss uploading the Physical or the Parent Permission/Concussion form during registration, you may log in back to your family account later and upload and/or complete the document at that time.

Follow these steps:

1. Login to your family account.

2. Find the recently submitted registration and click on "View" under "Reg Form" column. 

3. You may directly click on Step 4-Physical Forms on the left side of the submitted form.

4. Upload the scanned Physical Forms in File Upload.

5. Click on "Save" at the bottom of the page after uploading the files.
